Procedure are as follows:

1. Contact lab to request for lab sheet / work form.

2. Lab form will be sent to the requested clinic/polyclinics/hospital by the dispatch.

3. Fill up the details for the work as per required.

4. Contact Lab for collection of work.

5. Arrangements would be made to the day of collection.

6. Indicate the required date and time to clinic/polyclinics/hospital dateline.

7. Upon the lab work accomplishment.

8. The job will be sent back to the required clinic/polyclinics/hospital.

Download of Lab sheet / work form - click on file, right click and save the lab sheet. If you encounter problems downloading the file, kindly contact us.
