Procedure are as follows:

1. Contact lab to request for lab form.

2. Lab form will be sent to the requested clinic/polyclinics/hospital by the dispatch.

3. Fill up the details for the work as per required.

4. Contact Lab for collection of work.

5. Arrangements would be made to the day of collection.

6. Indicate the required date and time to clinic/polyclinics/hospital dateline.

7. Upon the lab work accomplishment.

8. The job will be sent back to the required clinic/polyclinics/hospital.
